offer. You will get the ability to have 2 active forms at one time
and have a combine respondent total for all forms of 12,000.
To buy an additional 2 forms it is a one time fee of $9.99 per 2. If you use more
then 12,000 respondents your monthly fee will be increased to $24.99 up to 40,000
respondents. If you exceed 40,000 we will contact you with further information on
payment.
Option 2.) With this option it will cost you a one time fee
of $99.99 with no agreement and $9.99 for every month of service. You will
get the ability to have 2 active forms at one time and have a combine respondent total for
all forms of 12,000. . To buy an additional 2 forms it is a one time
fee of $9.99 per 2. If you use more then 12,000 respondents your monthly fee will
be increased to $14.99 up to 40,000 respondents. If you exceed 40,000 we will contact
you with further information on payment.
